The following forms are provided as a resource to our visitors. If you need any of the following forms, we invite you to download and complete them, print them out and bring them with you when making your visit to our police department.

This form is to be used when requesting police records.
Click Here to download the 911 Special Needs Notification form (return by mail or in person to WHPD/ERS)

This form is used to alert the West Haven 911 dispatch center of any special needs you may have. The information is entered into
our dispatching system. When someone from the residence dials 911, the special needs are automatically indicated on dispatchers screens
to help aid police and fire and ambulance personnel to those special needs.

This form is used to update the police department's computer system with business owner and contact information. This information is
extremely valuable as it ensures the police have current contact information of a premises in case of an emergency (hazardous condition, break-in, burglary, etc.)

If you have received a bad check and wish to prosecute the offender, please fill out this form in its entirety and then return it to the police
department. Once on file, you will be contacted by a fraud detective and assigned a case number.

This form is to be filled out by persons who are requesting a handicapped parking sign. This form also includes guidelines and criteria
which must be met for the sign.

If you have received a parking ticket and wish to contest it, this report must be completed and turned into police headquarters. Please DO NOT
return the parking ticket with your dispute form.

If you have received an ordinance violation and wish to contest it, this report must be completed and turned into police headquarters. Please DO NOT return the ordinance violation with your dispute form.

If you wish to apply to the Public Service Unit, please fill out this application, have the signature page notarized, and return it to the West Haven Police Department. Applicants will be contacted by liaisons to the unit as soon as possible.

In 2007, the West Haven Police Department published a children's book. The book was distributed to area schools and libraries. If you are

Attention WHPD Retirees: The WHPD now has the Law Enforcement Officer's Safety Act application for qualified retired WHPD Officers. The attached packet has all the forms and guidelines available within a single self-executable archive. Once downloaded, double click the file to begin "extracting" the four separate files. These files cover the application, general orders, and qualifying standards. To download the packet, CLICK HERE.
